The Health Creation Network (HCN) is dedicated to fostering community health through innovative and sustainable programs. Our initiatives empower individuals, build connections, and enhance well-being across our communities.

Brave Dreams Project

The Brave Dreams Project is a pioneering initiative in Estacada that leverages art and creativity to foster community resilience and inclusivity. By supporting local artists and engaging students, the project bridges diverse groups and fosters a sense of belonging. Launched with a small team and a $5,000 micro-grant, Brave Dreams has grown to involve over 1,000 community members. This initiative not only enhances social capital but also inspires future community-driven projects by producing community art that strengthens cultural connections.

Asset Mapping

HCN’s asset mapping initiative is designed to enhance community health by identifying and leveraging local strengths and resources. This program connects individuals and organizations, fostering collaboration and efficient resource utilization. Through detailed mapping of community assets and extensive listening sessions, HCN has mobilized numerous untapped resources to support various health creation projects, promoting connections and resource sharing that enhance overall community well-being.

Health Creator 
Community Builder Program

This initiative captures and celebrates the stories of health creators, inspiring others to recognize their potential and contribute to community health. By sharing these stories, HCN fosters community pride and motivation. Dozens of stories have been collected and shared, increasing community engagement and awareness of health creation efforts. These personal transformations and community successes are highlighted through various media, encouraging others to become active health creators.

Strengths-Based Social Prescribing

Strengths-Based Social Prescribing links individuals with local services and groups that align with their strengths and interests, promoting holistic health and well-being. This initiative has enhanced social connections and improved health outcomes for participants by connecting them to community resources. For example, an Orchid Health patient with a passion for baking was encouraged to share her talent, leading to increased confidence, social involvement, and overall mental health improvement.
